Therapy Services

Homeward Bound Transitional Care is the dedicated rehabilitation unit of Baptist Health Nursing and Rehabilitation Center. It provides programs that maximize independence, which include a combination of physical therapy, occupational therapy and speech therapy up to six days per week.

  • Daily rehabilitation sessions – up to 3 hours per day – based on individual needs and abilities
  • Patient and family centered care team meetings that include therapists, social worker, dietitian, and nurses working together in collaboration with the physician and nurse practitioner to customize a plan to promote a higher level of functioning
  • Patient and family updates on the patient’s condition and progress
  • Through monitoring of patient weights, vital signs and medication, we are able to offer early problem identification.

Our physician, nurse practitioner, and nursing staff  provide the level of care essential to medically fragile patients requiring intensive skilled nursing care.

Special services are provided, such as:

  • Cardiac and pulmonary management
  • Orthopedic rehabilitation
  • Infectious disease recovery and management
  • PleuRx® catheter management
  • VitalStim® Therapy System for swallowing
  • Trachs, C-pap, Bi-pap
  • Stroke recovery
  • Inhalation therapy
  • Intravenous therapy
  • Tube feeding and nutrition management
  • Complex wound management and negative pressure dressings
  • Pain management
  • Suture/staple removal
  • Patient and family education to prepare for a successful transition to home

90% of Homeward Bound rehab patients are discharged back to their homes or to the community with varying levels of assistance.
